Gynecomastia

Gynecomastia is recognized by the enlargement of one or both breasts. The enlarged breast glands are palpable. An ultrasound can confirm the diagnosis. In older patients, a malignant disease should be ruled out.

Gynecomastia can be caused by hormonal imbalance, certain medications, or anabolic steroid use. It can also occur during puberty. Enlargement due to fat accumulation from severe overweight, without glandular enlargement, is called pseudogynecomastia (lipomastia).

Gynecomastia cannot be reduced by exercise.

The method depends on the findings. With pseudogynecomastia, liposuction is sufficient. True gynecomastia requires removal of glandular tissue via a small incision.

If pseudogynecomastia due to overweight, weight should be reduced before the procedure. As nicotine impairs wound healing, avoid smoking for a week before and three weeks after. Blood-thinning medications, such as aspirin, should be discontinued two weeks before.

Depending on the extent, surgery can be performed under general or local anesthesia. A small incision below the gland allows removal of tissue. Excess fat is suctioned, and the skin is tightened. A compression vest must be worn for six weeks. Avoid sun, sports, and sauna during this time.

Gynecomastia surgery takes about 60 to 90 minutes.

To ensure good results, wear a compression vest for six weeks after surgery. Physical rest is advised during healing. Also avoid sun, sauna, and sports for at least six weeks.

As long as no major physical exertion is required, you can usually return to daily activities after about a week.

Excess breast tissue is removed using scar-sparing techniques and very small incisions. Once healing is complete, scars will be nearly invisible.

Gynecomastia surgery by an expert is low-risk. Post-op tension pain can be managed with medication. Mild swelling and bruising usually resolve in a few days.

Recurrence of gynecomastia after surgery is almost impossible.
